The Airdrie Fire Department (AFD) and the Airdrie Professional Firefighters Association (APFFA) are committed to ensuring that your family is supported with dignity and efficiency during critical moments.
While this system was originally designed for Line of Duty Deaths (LODD), we have expanded its scope to include all emergency notifications, such as serious workplace injuries, sudden hospitalizations, or any non-fatal event where your family needs to be reached immediately. Relying on City resources don’t always accommodate for after hours or the most up to date contacts.
You will need your personal @airdriefirefighters.com account assigned to you in order to access this form.
To ensure your information remains accurate and under your control, the notification process is now self-serve.
Access: Members are granted access to the Association Private Google Site.
Maintenance: You can log in at any time to update your contacts, change your support preferences, or add new family details. Just look for the "Self Serve" option and you can update everything from your mailing and physical address, to your emergency contact / next of kin information so we know who to call if you are badly injured at work.
Incremental Updates: You do not need to have all the answers at once. If you are unsure of certain details (e.g., specific religious preferences or backup contact numbers), you can fill out the essentials now and update the rest down the road as you gather that information.
The Association maintains this data because it allows for greater flexibility and privacy than City-managed HR files.
Secure Environment: Your data is stored within the Association’s secure Google for Non-Profits workspace.
Restricted View: Information is only accessed by the Association Secretary or, in their absence, a designated emergency backup (Association President) during a verified incident.
Digital Only: To maintain security and version control, we no longer accept printed or physical copies of this information. All updates must be made through the secure portal.
When you access the site, you will see several fields. Here is why we collect this information:
Primary Notification Party - Your first point of contact (Spouse, Partner, Parent, etc.).
Local 4778 Member Support A specific brother or sister from the Local you want present during the notification to provide immediate peer support.
Secondary/Backup Contacts - Crucial if your primary contact is traveling, at work, or otherwise unreachable.
Children’s Information - Helps the notification team understand the household dynamic (e.g., preparing to interact with toddlers vs. adult children).
Personal Details (Religion/Birthplace) - While optional, this helps us handle background logistics for official statements or arrangements, significantly reducing the administrative burden on your grieving family.
